This agreement is between the Purchaser (“You”) and Documentation Wizard, LLC (Documentation Wizard) (“we,” “us.”)
Documentation Wizard® trainings (live and digital), forms and templates, and consultations, as well as anything and everything offered in the Membership Circle (collectively, our Materials) are designed for You to learn and implement the skills necessary to maintain psychotherapy records that accurately represent Your work and document medical necessity.
These Materials include any documents You may purchase and use including, without limitation, our trainings, forms, templates, packages, and any other digital products we may sell. These Materials also include consultations with Beth Rontal, any of her associates and consultants, or participation in and materials that are part of the Membership Circle.

Disclaimer
Using these forms and templates, participating in a live or online video training, receiving a personal consultation, or participation in Membership Circle and use of its Materials does not guarantee:
— that insurance companies will authorize the sessions You request.
— that You or anyone in Your practice or who You may supervise or consult with will pass an audit.
— that You or anyone in Your practice or who You may supervise will avoid an insurance recoupment.
— that Your documentation is immune to legal issues or regulatory complaints.
— that Your documentation will guarantee the fulfillment of worker’s compensation claims, disability claims, or any other claim or report You may need to write and provide.
The Materials do not purport to replace the advice of compliance professionals, nor supplant the individual therapist’s obligations to comply with legal, regulatory, clinical, and ethical requirements. Each therapist is responsible to use his, her, or their professional judgment and assumes all responsibility for clinical treatment and documentation for individual patients.
Refund Policy
There are no returns on the Materials. No refunds are given for any reason. All sales are final.
Coupons & Discounts
We may offer discounts or coupons from time to time. If You use a discount, You agree to pay the discounted fee shown during checkout. All other parts of these terms and conditions apply.
Limitation of Liability
You agree that Documentation Wizard is not responsible for any damages or losses You may face as a result of using or not using our Materials or participating in Membership Circle.
Personal Responsibility
By using our Materials or participating in Membership Circle, You accept full responsibility for Your actions and results. Documentation Wizard provides guidance and education but is not responsible for the outcome of your documentation, including legal, ethical, or financial consequences. You are responsible for your success or failure. Results may vary, and no guarantees are made.
Not Legal Advice
Our Materials are not legal advice. We are not lawyers, and our Materials should not be used as a substitute for legal counsel.
No Warranties
We make no guarantees about our Materials, including technical aspects. All content is provided “as is.”
Forms and Templates
The Clinical Forms have been developed to meet Medicare (Federal) standards. If you are a Medicaid provider, refer to your state regulations to verify requirements.
The Practice Management Forms, and Consents to Treatment Forms have been developed to meet Massachusetts requirements. Please read through the forms carefully and be sure You 1) fully understand them before using them and 2) adapt them to Your practice needs, and 3) have them reviewed by an attorney in Your state for state-specific compliance. The areas highlighted in yellow indicate where You want to pay special attention.
Neither Documentation Wizard nor Beth Rontal, nor any her associates or consultants are responsible for the content of Your notes, for passing an audit, or any possible legal issue that may arise in the course of Your work. By using these forms, You agree to release DW, Beth Rontal, and anyone associated with the development and marketing of the Materials from any and all liability for injuries that may occur in connection with the use of the Materials and to indemnify DW, Beth Rontal, and anyone associated with the development and marketing of the Materials from any third-party claims resulting from the use of these forms, including any financial responsibility for injuries incurred, regardless of whether injuries are alleged to have been caused by negligence in preparation of the forms.
AFTER YOU PURCHASE AND DOWNLOAD Your forms, please feel free to customize them to suit the unique needs of Your practice. For example, add Your name and licensure at the top of each form in the space indicated. However, the content of the forms may not be altered without express written consent. These forms are the intellectual property of Documentation Wizard and may not be distributed or shared with any other psychotherapist for their use. The copyright notices on the forms may not be removed.
Please make note of Your username and password so You can download Your forms or get access to the video training at any time by logging in again.
Membership Circle Terms and Conditions
The Documentation Wizard Terms and Conditions are part of this agreement. If anything in those policies conflicts with this agreement, the terms specific to Membership Circle will apply.
Program
We agree to give You access to the Documentation Wizard Membership Circle (“Program”). By joining the Program, You agree to follow all the rules outlined here. For a complete list of Membership Circle benefits, please visit the website: https://documentationwizard.com/membership-circle/.
Start Date
This agreement starts when You register for the Program.
Fees
You can choose one of the following payment options as detailed on our website:
- Monthly Membership: You will make monthly payments. The exact amount depends on the membership tier You choose. Payments will be charged automatically every month on the day you registered until You cancel.
- Annual Membership: You will make a yearly payment based on Your membership tier
.Payments will be charged automatically each year on the month and day You registered until You cancel.
Renewal Authorization
You agree to allow Documentation Wizard to automatically charge Your credit card as described above.
Cancellation Policy
You may cancel your membership at any time by following the steps in the membership portal. Access to Your membership will end when your monthly or annual membership expires.
Confidentiality
We respect Your privacy and expect You to respect the privacy of other members and of your clients. You cannot share any information from other members or from Documentation Wizard outside of the Program. You also agree to remove any identifying details when sharing client information during group sessions, in the Facebook group, Consultation Group, or any other Program events.
Submissions
Any content You share during the Program (videos, feedback, documents, etc.) may be used by Documentation Wizard in future versions of the Program. You will not receive any payment or compensation for this unless stated otherwise by Us.
Termination
We may terminate Your access to the Program if You violate the rules or disrupt the Program. No refund will be given if you are terminated.
Copyright and Trademark
All content, materials, and information provided on or through the Documentation Wizard platform, including but not limited to The Materials, text, graphics, logos, images, software, and multimedia elements are protected by copyright, trademark, and other intellectual property laws. The Content is the exclusive property of Documentation Wizard or its licensors.
By using the Materials under the license implied by Your purchase, You acknowledge that You may not sell, transfer, reproduce, distribute, publicly display, create derivative works from, or otherwise exploit any of the Materials. You may not post them on Your website, social media or anywhere the public can download or copy and paste them for their own use without express written permission from Documentation Wizard. Unauthorized use of any Documentation Wizard trademarks, service marks, logos, or any other proprietary Materials is strictly prohibited and may result in legal action.
If You wish to use any of the Materials for Your group practice or clinic, please contact Beth Rontal directly to discuss Your needs.
License
Unless otherwise stated, Documentation Wizard and/or its licensors own the intellectual property rights for all Documentation Wizard Materials. All intellectual property rights are reserved. You may view and/or print pages from https://documentationwizard.com for Your own personal use subject to restrictions set in these terms and conditions.
The Materials are licensed to the Purchaser for individual use only in the context of his, her, or their professional practice and use with clients. Copying or sharing the Materials is not permitted under the terms of this agreement.
You must not:

- Sell, rent or sub-license material from https://documentationwizard.com
- Reproduce, duplicate or copy material from https://documentationwizard.com
- Redistribute content from Documentation Wizard (unless content is specifically made for redistribution).

- Government agencies;
- Search engines;
- News organizations;
- Online directory distributors, when they list Us in the directory may link to our website in the same manner as they hyperlink to the websites of other listed businesses;

- Other Communication Methods: Where appropriate, additional methods of communication, such as in-app alerts or push notifications, may be used.
It is your responsibility to review any such amendments or modifications. Continued use of the platform after receiving notice of any changes will constitute your acceptance of the modified terms. If you do not agree with the updated terms, you must cease using the platform and services immediately.
No modification, amendment, or waiver of any provision of this Agreement shall be binding unless it is made in writing and signed by an authorized representative of Documentation Wizard.
Governing Law & Disputes
This agreement is governed by Massachusetts law. Any dispute arising out of or relating to this contract or the subject matter thereof, or any breach of this contract, including any dispute regarding the scope of this clause, will be resolved solely through arbitration administered by the American Health Law Association Dispute Resolution Service, and conducted pursuant to AHLA Rules of Procedure for Arbitration. Such proceedings will be governed by substantive (but not procedural) Massachusetts law, without regard to choice or conflict of law provisions or rules. If this matter is heard in person, the location of the arbitration shall be in Boston, Massachusetts. The arbitrator and not any federal, state, or local court or agency shall have exclusive authority to resolve any dispute relating to the interpretation, applicability, enforceability, conscionability, or formation of this contract, including but not limited to any claim that all or any part of this contract is void or voidable. Judgment on the arbitral award may be entered and enforced in any court having jurisdiction.
Fee Shifting
If there’s a legal dispute, the winning party can recover attorney’s fees.
