These Materials include any documents you may purchase and use including, without limitation, our Diagnostic Summary, Treatment Plan, Progress Note, Case/Collateral Contact Note, Discharge Summary, and Treatment Summary. These Materials also include any packages you may purchase and use including, without limitation, the Administrative, Diagnostic & Assessment, and Supervision/Consultation Packages, as well as our educational videos and consultations with Beth Rontal or any of her consultants.
Disclaimer
Using these forms and templates, participating in a live or digital workshop or training, or receiving a personal consultation does not guarantee:
— that insurance companies will authorize the number of sessions you request.
— that you or anyone in your practice or who you may supervise will pass an audit.
— that you or anyone in your practice or who you may supervise will avoid an insurance recoupment.
— that your documentation is immune to legal issues or regulatory complaints.
— that your documentation will guarantee the fulfillment of worker’s compensation claims, disability claims, or any other claim you may need to write and provide.

By using these forms, you agree to release Documentation Wizard, LLC and Beth Rontal from any and all liability for injuries that may occur in connection with the use of the Materials and to indemnify Documentation Wizard, LLC, and Beth Rontal from any third-party claims resulting from the use of these forms, including any financial responsibility for injuries incurred, regardless of whether injuries are alleged to have been caused by negligence in preparation of the forms.
The Materials are copyrighted by Beth Rontal and are licensed to the purchaser for individual use only in the context of his or her professional practice and use with clients. Copying or sharing is not permitted under the terms of this agreement.
The Materials may not be sold, distributed, or licensed by the purchaser for use by another individual or entity and any distribution outside of the practice of the purchaser is strictly prohibited. By using the Materials under the license implied by your purchase, you acknowledge that you may not sell, transfer, assign or otherwise distribute the Materials. You may not post them on your website, social media or anywhere the public can copy and paste them for their own use.
There are no returns on the Materials. All sales are final.

Dispute Resolution
