Customizable Documentation Templates:
Available as Word Docs, Fillable PDFs, and static PDFs. Word Docs and fillable PDFs can be customized to meet your clinical needs. There is no need to decide before purchasing. All three versions will be sent so you can decide what works for your practice. And remember, the forms can always be downloaded again at any time from your account.
Word Docs use MS WORD Templates to lock the formatting in place while providing plenty of narrative space that expands to meet your distinct reporting needs. Useful dropdowns for CPT Codes and Severity attributes save time and reduce errors. The option to use a date picker for all dates and dropdowns for times also enhance the ease of use for these forms. Checkboxes are incorporated to remind the user to answer often repeated responses such as risk assessment or modality. For best results, we recommend using the most recent version of Microsoft Word in Microsoft 365.
The fillable PDFs include all the same attributes as the WORD Docs and a few more such as automatic phone number formatting. However, their format is static and does not expand for long narratives. The narrative spacing is created based on Beth’s years of practice. If you are significantly technically challenged, these may be the forms for you. We recommend using the free Adobe Reader with the fillable PDF forms.
Use the static PDFs (non-fillable) if you’d like a pre-formatted version that may more easily print for you. This is a good solution if you do not use Microsoft WORD. We recommend using Adobe Reader. The PDF may not function correctly in a browser.
You are welcome to make minor adjustments to the documents to suit your clinical needs, but you are NOT permitted to share them with others or remove the copyright.
